If you are requesting accommodations for a test or exam,
in most cases you can use online registration:

However if your request for accommodations falls under one of the following categories, use these links instead:

Test/Exam Date Registration Deadline

May 1, 2013 - August 12, 2013 14 days' notice *

August 13, 2013 - August 16, 2013 Monday, July 29, 2013

* 14 days' notice means that your registration form must be complete and submitted a minimum of 14 days before the earliest test/exam date listed on the form. So if your test/exam date is the 15th, the last day you could submit your form would be the 1st.

Please be aware that accommodations are not guaranteed to students who request accommodations after the deadline
In order to register for a test or exam with accommodations, students must be
(1) registered with Accessibility Services

(2) authorized for test/exam accommodations by an advisor at Accessibility Services
