PDF Forms

Most registrations are done online. However if the registration instructions indicate that you are to use the PDF form, download it with the link below.

For each test or exam for which you require accommodations download & complete a registration form (one form per request!) and follow the submission instructions on the instructions page.

MAC users:

The default PDF viewer on many MAC's is 'Preview', which may not work for the input fields on the forms

Try using the viewer embedded in your internet browser (ex. firefox) or, for the most reliable results, download the form to your computer and make sure it opens with Acrobat Reader

You can either change your default viewer for PDF's to Acrobat Reader, or open Acrobat Reader first and drag the form icon into Reader to open the file

You can fill out the form on your computer and re-save to email back to TES. No printing!

Problems?

If you can't fill out the forms on your computer, or if they look wrong when you try to fill them out:

1) make sure you have the latest version of Acrobat Reader

2) DOWNLOAD the forms to your computer and open them with Acrobat Reader. If you just click the link and it opens in your web browser, the fill-out fields may not work right. Especially if you are using a MAC. Save the form, and re-open it, making sure it's opening with actual Acrobat Reader
